I have a long list of options that can be chosen and then submitted.
I have figured out howe to Move forward/back in the recordset, but how to I keep track of what the user has 'checked' or unchecked, so that on submission of the form I get the final list of options selected?

You could store them in a cookie, session or database every time a "next" page is loaded. Any would work, but session would probably be easiest.

the problem is how to get the data into a cookie or session or array from the form.
How can I get the value of the checkbox into javascript in order to get it into php?

I would put a script at the top of the page, that process all of the form data. store the information in a serialized multidimensional array. When you want to add to it, just pull what you already have from the session, unserialize it, add to it, then reserialize it and put it back.
