Simple Vendor Database

I would like to know if there is a script that is available that will allow someone to add edit and delete vendors.

I dont want it to do too much....

just simple:

Vendor Name
Address
City, State
Phone
Site

Thats all

and have it store in a database or text file..

i hope that someone can help me, for i would really to find or have one create.
