Does anyone have or know where to find a php/mysql based online form with the ability to upload files in the process.
i need to create a form here at work were people can upload .txt or .rtf files only.

Theres lots of sites. Your gonna have to read through a lot of them, before you find the one for you.
Your other option is to create your own. It isnt really that hard either.
yeah, i took a look around and found a great script that creates the form for you with the option for uploadable files too.
