Logging IP's for queries

I would like to log the IP's of those that submit queries on my database. I'm a newbie and need a commented script to assist me with configuring it for my own use...can anyone point me to a snippet of code that would help me set this up???

You can use $_SERVER['REMOTE_ADDR'] that will return the IP address of the user who is performing the query
