How to get confirmation back from emails?

If I send an email to a user requesting confirmation, how do I recieve the confirmation back?

Lets say I send an email with "Click here to confirm". How do I keep track of who has clicked the link?

Depends But it all comes to the same way..

There must be some sort of Identification using a database.

So eg..

to actiavate an account
URL could look like this

domain.com/activiation.php?id=1

That way we know the activation id is one. And there username could be bob etc.

You keep track of the information you send out, probably in a db. The link they click is unique, and passes information to a php script that marks that entry in the db as "confirmed."

this is get vars $_GET or $HTTP_GET_VARS which would be more accurate to your terminology
