Need help with coding - form wont send

I am using PHP NUKE and I added a new module, I took the feed back form and converted it to a Gaming Comunities join Request form

every thing went fine, i had a few errors I worked out, but then this morning she stoped sending emails to me.

If some one can look it over or point me to a PHP Nuke frendlier site I would be thankful

what is happening is after you hit send it cycles back to the join input screen and does not send the email, it act like there is an error on the page or there is missing fields
I've been looking and need fresh eyes
