Create checkboxes from DB info? (VERY newbie)

hello, I am very new to php and am having a really tough time fishing through search results. If someone could please help, that would be grand.

What is the best way to populate a check box set with information from the database?

example.

This is for a retirement home referral service. I have a table with considerations like kitchenette, nurse on site, yard, etc...
(the table is like this)
consideration_id ....consideration
1 ............................kitchenette
2.............................nurse
3 ............................ yard
I need to be able to search for a suitable home using the matched criteria from these check boxes.

well you have a form which posts to a process file.

Now the check box values are 1 for on and 0 for off (selected and not selected)

So..

SELECT * from tbl_name WHERE box1 = "1" AND WHERE box2 = "0";

Or whatever
