Simple Affiliates Page

Right now my site is just a big forum. This is fine and all that but I need a page to place affiliate forums/sites on. So how would I go about creating a somple affiliates list page? I was thinking about making a small form the admins could use to fill out and then it would automatically add the website they filled out to the affiliates page. It could be using a MySQL Database or a text file. Please go into some detail as I have never worked with MySQL Database before and am quite a beginner with PHP (I know most of the basics though). This site is a good place to begin learning MySQL/PHP. Once you've got the basics down, you should be able to create a simple form that would allow an admin to enter information such as a link, etc. If you have any problems with coding, please post your questions here.
