MySQL Table Fields

I would like some advice on designing a database. Say I have several Forms that wll go with the same job but not every job will have several forms some will have 1 and some will have up to 6. Should I name the fields. form1 form2 form3 etc...
or should I have code that will insert a new row for every form. This DB will not be widely used or never grow very large.

As long as you have some sort of unique identifier(like job name or number), then insert a row for every form
