Help with creating a complex Email Form

Kay, I'm a major newbie when it comes to PHP. So if you have the answer to my question, please tell me one step at a time so I can understand.

But anyways, here's my situation. I want to make an order form that includes:

1. Name, Email, Phone, Fax, Address, & etc. imput area.
(Make name, email, & phone a required field that you have to fill in. Otherwise it wont submit.)

2. Option 1, Option 2, Option 3, & etc. drop down menu.

3. Option A, Option B, Option C, & ect. checkboxes.

4. Option X, Option Y, Option X, & etc. radio buttons.[/COLOR]

And I need all of this to somehow be sent to my email account...and make scence.

Please help me...I feel so lost and stranded, because there's many tutorials out there, but it's either too complicated to understand, and or doesn't contain the information that I need.

you need to start off by creating a form, i usually find in my laziness that dreamweaver/frontpage is good.

remember that for the radio buttons the name tag has to be the same for all of them or else they wont work

for the message of the email its very simple as you just grab the info posted by the form to get all the info so:

textbox
<input type='textbox' name='1'>
to get the info you do:
$_POST['1']

radio button
<input type='radio' name='r' value='r1'>
<input type='radio' name='r' value='r2'>
to get the info you do:
$_POST['r'] -even though there are multiple radio buttons this will get the value of the one chosen

i will post back with a checkbox solution as i have never really used them :o
