[Resolved] php, dsn, ms access, adding data

After following the tutorial about entering data in an MS ACCESS db and with a slight modification, I have managed to stop the type mismatch error I was given

However when adding data to the table, the index autoincrements as expected, however firstname, lastname and phone number are not entered into the table in the database

I've searched for the answer on this forum through the search engine but I have a long way to go before I exhaust the returned threads and yes I have tried to narrow the search.

So Can anyone help?
