PHP Formmail Problem/Question!!!

Can I add two formmail php scripts to one webpage?

For example this one below goes to me from the form submission!

2nd Form, but can you guys help me with the code so not only can it send this email to the person that filled out the form, but to attach a word document from the server. Lets say its @ http://www.website.com/attachthis.doc
