Simply mail form code.

I need a simple php mail form code for my site. Right now, I'm looking at a form for affiliation request, to report an error, and to contact me. I've been referred to a couple places, but I'm brand spankin new to php. All these scripts and such, I'm not familiar with at all. Where should I look? Regardless, I attempted to install a script from Dodo's Script Collection. You can view my horrible attempt here. any suggestions? Thanks!!

you don't need a form, you need a script to handle it. the form is just html code, unless you need things dynamically inserted into the form.
