Form Help

I have been thinking of this for a couple of days now and at last decided to do something about it hence I posted here...

I have a form I have created using html and need to post the contents of the form 1) to a database and 2) an email to the person who filled it on.

On the form I have a field which requires the user to click on ticky boxes (? - is that the professional name for them?). I am wondering how I would pass this field to a database/ email to send/ store. In some cases, there may be several boxes ticked.

I have named the boxes as an array called equipment[1] to equipment[6]. Has Tubbs done well? Is this correct. All the books I have bought seem to skirt the ticky box question and only list the usual form elements like text boxes, drop down menu's etc.

Is there anyone who could help me?

There's a checkbox (technical name) on there. Try both submitting with and without checking the box to see what the response is.
