email questionaire

im kinda confused on how you would make a form that mails the data...

how would u make it gather the data, and input it into the email in certain places?

Like, you clicked 'send' and you got an email saying:

"Thank you, Gordon L. N00b , for signing up to DXL.net.
Your password is: frankfurter
Activation Code is: 870 "

how would u do this?
