Add a site (URL) Riciprocal Link Form

Does anyone know how or where I can get a PHP script for an "Add your site Form?"

Try this http://php.resourceindex.com/Complete_Scripts/Link_Management/Free_for_All_Links/, and when you can find anything that usally means you have to code it your self (which in this case shouldn't be too hard 🙂 )
