I know how you feel, I went thru this in July of last year.
Try the phpbb dot com site, check the support forums there,
and read the manuals/README/NOTES.
However...here's what I learned:
DSN: 9 out of 10 times, it's "localhost". The mysql server is
running on the same machine as the server, so try this.
DB Name: The physical name you used, "mySQL".
DB Username / DB Password: You said you have these.
Prefix for tables: Sometimes, we put multiple forums and other
stuff inside one database. Having a prefix keeps tables from
getting mixed up. I suggest "phpbb_" for your first forum.
Admin Email: A VALID address that the software can use to send messages to you. I use "forum-admin@..." to alert me that it's
from the software.
Domain Name: Whatever your site's main name is. Try it without
the "http://" first time, and no trailing slash.
"www.whatever.com"
Server Port: Usually port 80, so leave blank. IF you are installing
a hidden forum accessible only thru a certain port, that port
would go here.
Script path: A sub-directory name where the forum scripts will be
located. I used "forum", because I installed the files at
www.whatever.com/forum/. Keep it close to the root, as there
are DEEP sub-directories.
Admin Username: A new user name, internal to the forum, used
for setup and maintenance inside.
Admin Password: New password for above, MAKE IT DIFFICULT
TO CRACK, and DON'T use it anywhere else.
There will also be a listbox with different database types,
I had to use "mysql4" to make things work.
Again, read the manuals/READMEs/NOTES for things that may
be different from my installation!
