users profiles and Uploading

Hi, i have some php uploading codes, but i need to know how users can send their profiles(address,phones,name,etc) with the uploaded file. I am running a photo printing store and i nedd to know who is sending each photo for printing.Their profiles are in a database but how can i make the upload program to read the user profile and send it with the uploaded file?.

You can ask for additional information when you prompt for the upload; just hit REPLY to this note and pay attention to how the message board software does it. The form should be

enctype="multipart/form-data" method="post"

You refer to $FILES[] to access the upload information, and $POST[] to access other form fields.
