PDFlib and FDF

hi,

I am doing a form for an insurance company, and they want the results to be in a PDF file that looks like this with the users input:

www.coverageglossary.com/forms/Acord_CLApp.pdf

my client cannot install PDFlib or FDF on their server, what can I do to get around this. and is there a way i can take a PDF file and add text to it so i can turn it into a template?
