Hi people, can anyone help me with this please???

i'm wanting to make an order form on my site to purchase 1 product, i need it to have sections for :-

1, fullname, (text box)
2, address, (text box)
3, postcode, (text box)
4, email address, (text box)
5, how they would like to pay, (drop down box)
6, how they heard about us, (drop down box)
7, and choose a gift, (drop down box)

they need to be checked so no info is missed out and
all these details need to be sent to "MY" email address and also to be stored into a mysql DB...
plus a conformation needs to go to the customer.

Is there anyone that can help me achieve this or help me out at all???

and: it would be just to nice, if you'd use more specific topics (not "i need help" or something like that).
