encapsulate your strings with ticks and that should take care of some issues...

Actually, one separates the various name=value pairs with commas, not "and".
The revised code would be:

$query = "UPDATE `staff` SET
	`first_name`='{$first_name}',
	`last_name`='{$last_name}',
	`faulty_id`='{$faulty_id}'
	`date_started`='{$date_started}'
	`group_id`='{$group_id}',
	`module_id`='{$module_id}',
