connection to database

i was wondering if anybody can help me. I need to connect to a form to mySQL database. I have Frontpage, My hosting plan comes with a couple databases and a phpMyAdmin link inside cPanel X, and a My SQL Account Maintenance. I'm really new and i'm wondering what to do to link the form in frontpage to the database (if thats what your supposed to do) so i can see the data. I believe i'm using Apache version 1.3.29 (Unix) and php version 4.3.3. Thanks
