Looks like it's still in install readme for the windows installer.
Is it still accurate that if you're in a windows production environment, running IIS, you should run PHP as a CGI rather than an ISAPI? Are there still issues and stability problems?

I haven't done much reading on the subject but as you've had no resposes yet I'll keep you company. AFAIK there are still issues with PHP as an IIS ISAPI, but I don't know of any major issues using PHP as an Apache module under windows.
I haven't seen PHP have problems as an IIS ISAPI module for about four years now - admittedly my acquaintance with PHP on IIS has only been sporadic since then.
Most people I have spoken to have said to use the CGI version. Whether they have experience with the ISAPI version or are just passing on what they have heard, I don't know.
