CSV into MySQL

hi everyone i have got the following csv file...
mms3_data.csv
it opens in MS Excel
but i want to use it in a mysql database so i can display the results up on my website.

load data infile 'filepath'/filename.csv'
into table tablename
fields terminated by ',' optionally enclosed by '"'
lines terminated by '\r\n'
(
column1,
column2,
column3
)

i was given this and i think i have 2 change the filepath and edit the

column1,
column2,
column3

part,

however filename is fine but the column part has me stumped the file i am talking about can be found here

the columns are the names of the columns in the db...just make them match to those columns in your database
