simple input console

hi,

i have a hard time making a input console for mysql using PHP,
i already have my PHP, Apache, MySQL and PHPMyAdmin runnig on my server. My scenario is im making a simple form with a text boxes and a submit button im trying to send a data from the textbox to the database, but i am not successfull.

is there anyone that have a simple code that have a visible form with textbox and submit button and able to submit data from textbox to database, using php script.

i need your help!! thank you in advance!!!
