Convert 2004-05-10 to 10/May/04?

Date conversions have always been my misery, so if someone could help me with a function to convert Convert 2004-05-10 to 10/May/04, that would be spectacular.

Essentially, I will be taking a date in the 2004-05-10 format, converting it 10/May/04 format and the splitting it into individual vars ($day, $month, $year).
