connecting to external MySQL DB

I know its possible, but I dont know exatcly what I have to do. First I know I have to change permissions so the Database can be accessed from outside localhost, but where/how would I change that? And after that is changed, what would be the approriate substitute to fill in for "localhost" in mysql_connect()?

Ive searched the forum but I havent found anything besides "change your permissions". If you could explain in semi-detail it would help me out a lot. Thanks in advance

Ive searched around and people told me I should input the IP for the hostname. I don't have a 2nd server yet so I can't test it out for a week, but does this putting the IP for the hostname work?
