Preamble: I am a programming-idiot beyond basic HTML, but I do have a small capacity to learn. What I am looking to here is just advice as to what is the best way to go about setting this up(someone told me php so that is why I am here). In the example below, it doesn't have to look like this, but anything similar.
I run an online video game league for the PS2 NCAA Football 2005 game. In it, we have a vote (by all coaches) on who is the Top 25 teams of the week. They seem to have a hard time doing this if they are required to post their Top 25 on a message forum, so we are trying to find something easier to do. Basically, we want this:

How that works is there is a complete list of all teams (ours are different) and you sort them by moving them to the right. From there, you can sort the teams on the right by clicking the up and down arrows to put them in a proper order from 1, 2, 3, etc.
Now, I have broken my wishes down into two scenarios.
Dream Scenario - Not only do we get this interface, but the server tabulates the data for me (awarding 25 points for each 1st-place vote, 24 for 2nd, etc) and emails me the results when I am done with the poll.
Other scenario - When the user hits submit, it emails me the results a la a standard cgi web form and I'll tabulate the data.
