Web Application/Authorization Request Form

Im am very new to this type of stuff. I am trying to create some sort of web application that will have an online account request form of some sort that a potential new user will fill out. Just a standard form with things like name, and stuff like that along with the thing they are trying to gain an account on. After that maybe an email sent to administrator who will login and authorize that user account access. I am wondering how I would implement this. Maybe there is already an application out there to do this or something. Any assistance or maybe places, technologies to look at would be wonderful. This would then update a server adding the new user to a database of allowed users. This seems to me kinda like a phpBB that I did in the fact of authorizing new users and activating account. But I am looking for a purpose buit application to apply for an account, adminstrator grant access, and then update a mysql database. Thank you.

If your wanting to create this yourself id try and get well equiped in MySQL Update, Select, Delete functions, as well as many of the PHP Basics.

However if you want a pre-built solution there are many out there try searching http://hotscripts.com to find them. Some will probably cost some money, others will be free. But you will need to test a few to see what you like.
