multi answer questionaiire

I want to put a question form on my site where the user can select answers form a form and then the script will send them the answer according to what they answer eg

What is you hair like?

Option a) Soft

Option b) Long

Option c) Dry

Option d) Bald

If the user selected ansers A and B they will get a different answer to if the user selected A and C.

any help on this would be great 🙂
