building forms with checkboxes

can someone tell me what the best way is to make a form with checkboxes
i mean the user has to check the checkboxes and the form will build with the corresponding form modules

example:

checkbox1: catering form
checkbox2: p&o form
checkbox3: library form

when the user checks 1 and 3 the form will build with form 1 and 3
and a submit button

if you want the form to be created instantly, that is to say before the form is submitted then this cannot be done with PHP, you must use javascript. if you want to use PHP to build the form then i suggest wrting a function that takes the form type as its argument and then spits out the appropriate HTML. when the form is submitted simply call the function (or make multiple calls) based on which checkboxes the user checked.
