form question?

I am trying to do a form with a file upload button ( I want my guests to be able to send me a photo) but I am not having any luck. 🙁 I got the typical form script but can't seem to get the upload button to work. 🙁

does that make any sense what so ever? :lol:

Any help would be appreciated.
