PHP forms talk to mysql

Ive been asked to build a online diary which enables users to record their feelings about training courses they ahve participated in. Im feel overwhelmed by the processes involved here. I know my MySQL needs to be built first but i was looking for some pointers or tutorials on building forms using PHP that store/retrieve info from a MySQL db

You can probably get a free script already written and just modify it to suit your needs:

Here is what you need to do in a nutshell:
1. have a script that allows users to insert their comments into the db
2. have a script that retrieves the comments from the db and prints them on the page
