small help

Hi All,
I really really need your help. I have created a survey form in html with radio buttons, text buttons etc.
I also added a submit button which is suppose to send me the survey once it was filled via email (php email form).
I know how to make the mail.php script to send me the text buttons but how do i make it send me the chosen radio button??

example:
let say i have a question such as:

How satisfied are you with the service?
and i have the following choises
(radio button) Unsatisfied
(radio button) satisfied
(radio button) moderate
lets say he chose satisfied.

how do i make it in the scrip that he will send me his choise?

i hope i am clear. :/
