Need routine to determine what the page name is

I am looking for a script that can retreive the current page name. This will be passed to a hidden field in a form that will tell the site adminstrator what page a user came from when they clicked on a "contact us" hyperlink. Any ideas? Thanks.

$SERVER['PHP_SELF'] or $SERVER["SCRIPT_NAME"] - just for the filename
$_SERVER['REQUEST_URI'] - for the filename with url query

but u should get that info before your visitor entered to your "contact us" page.
