ABC bookmarks with mySQL call

Please excuse me if this is a repeat question, but I've searched everywhere.

I'm trying to add A-Z bookmarked links across the top of a page that lists all my active database rows alphabetically.

I can call up everything fine, but how do I add the html bookmark code for each letter within the text? Is there a way to incrementally go through my results letter by letter, adding the bookmark code, and continuing without slowing the page load to a crawl?

I want the page to have this across the top:

A B C D E F G ... X Y Z

with each letter hyperlinked to an anchor below that corresponds to the authors last names. When the visitor clicks on the "G" it takes them down to the first of the G-names. The html code would look like:

<a href="thispage.php#G">G</a>
.
.
.
<a name="G">

So my question really is, how do I dynamically place the <a name= > into the correct location within my alphabetic string?
