Well, I went to the author's website, and he had 2 versions available for download. v 1.21 and v 1.0
Both of them had a demo.php file. You can refer to that file to see how it is implemented in action. Also, I realize you are new at PHP. You should get into the manual at php.net, and look up some tutorials (Google something like "PHP Tutorials") so you can begin to gain an understanding. A Poll system is a good easy project to get started with.
