My first form

I have a MySQL database with the following tables

Journalentry (Entryid,participantreference,Trefid,Tresponse,Grefid,Gresponse,Date,Timein,Timeout)

Trainreflect
(Trefid, prompt)

Genreflect
(Grefid, prompt)

Im having trouble building the form for this- ive tried several tutorials and keep getting stuck.
The form is like a journal entry which must store responses in training reflections (tresponse) and general reflection (gresponse) when the user is prompted by a number of questions such as:

How did you feel as you arrived to the training session?

How did you feel during the session?

Did any of the activities provoke a strong emotional response from you?

Did any of your feelings surprise you?

How did your react to your fellow participants?

What did you learn about yourself?

What did you learn about Counselling?

What did you learn about your own skills?

What happened today?

How did it make you feel?

What did you learn about yourself?

How have you practiced your skills? How has this gone?

To save space in my db the prompts will be assigned variables such as $prompt1= "How are you"

How can I do this form- please help!!

Assuming that you want people to enter a meaningfull answer, and assuming that the list of questions is static:

How did you feel during the session? &nbsp &nbsp<input type="textarea" cols="40" rows="4" name="q2" tabindex="2"></ br>

...etc

Of course, the format is not included here, except for the &nbsp. You can either use a table for layout, or CSS with class to make it look nice.

update.php will receive the input as an array which you access through $_POST[q1] etc and enter into your db through an sql insert query.

Thx for the reply. What does the $nbsp stand for and how what is needed in update.php?

&nbsp stands for 'no back space' and it stops the browser from removing spaces, or forces it to include a space so that you get a decent gap between your question and the text box.

You need an insert query. Look up the manual, or more rudely RTFM http://dev.mysql.com/doc/mysql/en/INSERT.html

$sql = "INSERT INTO (whatever your table is called) VALUES (" . $_POST['q1'] . "," . $_POST['q2'] . "," (and so on for all questions and answers)

Obviously, you will also have to post some sort of user id to update.php along with the answers, and any other required fields. Read the manual. Make an effort. Write some code. Get some errors. Them come back for some more help. It is the only way you will learn and improve.

Or else pay me £50 per hour to write all your code for you.

Actually,   is the character entity reference for "no-break space" (or "non-breaking space, if you prefer).
But that isnt important in PHP.

Ive created a form (journalform.php) and an update page (journalentry.php)

When I test it my database is not populated:

Can anyone spot any errors in my coding? Maybe I have a local server issue

echo your query to the screen, I think you'll find that your values are included as unquoted strings: your query will fail. I'm also assuming that you have just included bits of the journal entry script so I'll just guess about that
