the vast majority of people who help others on most forums like this do so to help people LEARN.
its sounds like your asking someone to write the script for you. even though you said "help me", you have made no attempt to try it yourself. people usually interpret this as "do it for me"
if thats the case, maybe you should bring some money to the table. or if you dont want to pay someone, then make an effort to do it yourself, and then ask for help when you hit a snag.

---depends upon your skill level. its not difficult at all for someone w/ moderate php ability.
and to give you some direction
after you collect the data and validate it, you could store it into session variables. then provide them w/ the option to finally submit it. when they "submit" it for real, then use the info you stored in the session vars to send your email and do whatever else you need to do....
