I am getting duplicate records inserted into mysql

I and others have been experiencing the following problem when accessing my website. When a record is inserted it is inserted
twice. Is there an issue with WindowsXP with or with certain
patches/updates?

Someone else had this problem. I've searched for the thread but for the life of me I cant find it. Maybe you will have better luck. It was this year some time.
