Member Area Mayhem

I've been trying to build a simple Members Area on a website.

I've downloaded and tried many scripts, but always manage to run into a dead-end.

I finally decided to follow a tutorial on building a script from scratch.
It feels like I am very close to making it work.

Here is the problem I now have:

When I go to the "Sign-up" or "Register" page found at:
http://www.menloparkrotary.org/database/register.php
I'll fill out the requested Username, Password and email address and submit.

Upon submission, I get this warning:

Warning: Cannot modify header information - headers already sent by (output started at /home/content/m/e/n/menlo/html/database/register.php:1) in /home/content/m/e/n/menlo/html/database/register.php on line 61

What is this?!?!
