Well, let's think it through.
At some point, they've said, "I want to join every site you have and that's a fact, you can use my info for everything".
So, they checked some box.
Write it down.
Then, when they come to a similar page again, if you can identify them (do they log in? did you set a cookie?), you check wherever you "wrote it down" (file? db?, etc., etc) and if you can find a match, you either marked the new checkbox "checked" or you shoot some message to them like "I know you, and you're signed up."
So far, you've posted one line of HTML. Take my thoughts above, work out a plan that fits the situation, write some PHP, and get back to the forum ... I'll bet ya someone can help you out. 😉
