File Upload script failing... worked before...

I have a simple file upload script - I've written hundreds of these in the past.

When the form to select the file is submitted (it submits to itself), it says the page cannot be found.

I tested the same code on a different server and it functioned fine.

The script was functional before and just recently stopped working.

There have been no server updates performed, nor have I changed any of the code in the script.

Does anyone know what could be happening here?

All other file upload scripts that I have still function fine

Any ideas?

i did try restarting apache and PHP to no avail... I'm at a loss here

Update:

This is actually occuring with any form on the server that includes a file upload (<input type='file' name='userfile'>).

I've put in a request to have apache upgraded to the latest stable version, which will probably take a solid month

Does anyone have any clue as to why this would suddenly start happening?
