Does anyone know how to debug / fix this type of error:

Warning: (null)(): Failed opening '/home/user/public_html/phplive/index.php' for inclusion (include_path='.:/usr/lib/php:/usr/local/lib/php') in Unknown on line 0
This error popped out after trying to login to phplive. It has been installed and working fine for several weeks without any previous problems, and we are running on a linux box.
Kernel version 2.4.21-9.0.1.ELsmp
Apache version 1.3.29 (Unix)
PHP version 4.3.4
The error persisted for several login attempts and then went away.... 😕
