Need help selecting specific records

I have a page where users can select records based on different criteria from drop down lists and need to add another selection criteria as a drop down list.

The new table is assessor and the field I need to display in the drop down is a combination of firstname and surname.

The resulting selection should show all records that are associated with the selected assessor.

The current code is below. Any help would be greatly appreciated.

if you've created the form for users to select their criteria, surely you can add a new drop down box yourself??

or is your question more specific?

You're correct. If I created it myself then I wouldn't have a problem. Unfortunately I didn't create the page myself. I'm not that smart! (Wish I was)

That's why I need some help. I do have an understanding of what the page does but I don't know how to create the additional drop down list based on all the assessors in the database. Currently around 500.

Is that too many for a drop down list?

AFAIK there is no actual limit onto what you can put into a dropdown. however you might find 500 or so is a built unwieldly and not user-friendly. i'm assuming you know how to create a dropdown list (if not seek help in an HTML forum not here).
to create it in PHP, you'll need to extract the data from the DB and loop through it.
