agh! pls help, click buttons and flash and php

hello

i have a form that uses the below php, this form works perfectly... what i need to do is add to my flash form two click boxes, 'yes and no'... but i don't know what to add to either the fla buttons of the php code below... pls help as i have a deadline and am not sure...
