Make an HTML page with a form like this:

Send this file: <input name="userfile" type="file" />

Make a file called 'upload.php' with the contents like so:

$uploaddir = '/home/yourName/public_html/uploads/';
$uploadfile = $uploaddir . basename($_FILES['userfile']['name']);

echo "File is valid, and was successfully uploaded.\n";
} else {
echo "Possible file upload attack!\n";
}
echo 'Here is some more debugging info:';

Now, you'll have to edit the "uploaddir" path to fit your server's environment, but that's basically how to upload a file and move it to the appropriate location.
