[Question] about Submit by PDF-form

hi all
I'm a php-newbie,I have some questions about PDF form below:

I created a PDF file and put a form in it , i wanna let somebody to fill the form and submit the data to my web sever with PHP-enable , but I don't know which is the data's type that submited by PDF file, and I wanna know the PHP-code that recieve the data from PDF's submitting...
