how to build a form to upload pictures w/text to mysql database

i have a simple form to add records and display them from a mysql database. what i really need to be able to do is to add (upload) pictures to each of the records i add and also to diplay the pictures that correspond to a certain id.

please help.....

You have two options:
-- Upload photo as Blob binary (photo stored in D😎
-- Store path to image in DB and upload photo to folder

Which would you like to use?
