HELP: slide bar to select an option on a range of 1 to 10

okay, so you know how on some surveys they give you a range from one to ten, where one is really bad and ten is really good. well, i am trying to code something like that where it is an arrow that you can move along a bar to select a number within that range. does anyone know if this can be done with php? and if so, how? any ideas would be really helpful.

Well, techincally, the bar's value (what the user selects) can be processed by PHP, but making the bar and displaying it the user is HTML's and JavaScript's job.
